چکیده

Symptomatic Disease Bone disease in its florid form, osteitis fibrosa cystica generalizata, is now rarely seen. In 1947 Norris (1) found an incidence of this condition of 91% in those with a parathyroid adenoma. The earliest changes can be detected by X-rays of the hands where subperiosteal erosions can be detected in the phalanges (especially radial aspect of the middle phalanges) and terminal tufts. The skull demonstrates a mottled appearance with lucent cystic areas ('pepper pot skull' in its most florid form) and any bone may demonstrate cystic lesions due to osteoclastomas or brown tumours. These lesions produce skeletal pain and may lead to pathological fractures. Subtle bone changes are rarely detected on skeletal survey and there is little justification for radiology to aid diagnosis unless there

Hyperparathyroidism in multiple endocrine neoplasia type II A Mini - review

Primary hyperparathyroidism (PHPT) in multiple endocrine neoplasia (MEN) 2A occurs in only 15-30% of patients. It is rarely the first feature recognized in the syndrome, is generally mild and is sometimes expressed only as parathyroid tumors discovered during surgery for medullary thyroid carcinoma. A predisposition to MEN 2 is caused by germline mutations of the RET proto-oncogene on chromosom...

Neonatal Seizure as a Manifestation of Unrecognized Maternal Hyperparathyroidism

Maternal hypercalcemia suppresses parathyroid activity in the fetus resulting in impaired parathyroid responsiveness to hypocalcemia after birth. Resultant hypocalcemia may be severe and prolonged and rarely may lead to convulsions. Here, we present a newborn infant admitted to the pediatric emergency department at age two weeks with recurrent tonic convulsions due to asymptomatic maternal hype...

Clinical review 122: Parathyroid carcinoma.

Parathyroid carcinoma is an uncommon cause of PTHdependent hypercalcemia. The collective published experience with this rare neoplasm has provided a distinctive clinical profile that differs in a number of respects from that of benign primary hyperparathyroidism (1–3). The distinguishing features of parathyroid carcinoma assume even greater prominence when viewed within the current context of p...

Primary Hyperparathyroidism in Patients with Multiple Endocrine Neoplasia Type 1

Primary hyperparathyroidism may occur as a part of an inherited syndrome in a combination with pancreatic endocrine tumours and/or pituitary adenoma, which is classified as Multiple Endocrine Neoplasia type 1 (MEN-1). This syndrome is caused by a germline mutation in MEN-1 gene encoding a tumour-suppressor protein, menin. Primary hyperparathyroidism is the most frequent clinical presentation of...

Neonatal hypocalcemia due to asymptomatic maternal primary hyperparathyroidism.

Neonatal hypocalcemia resulting from maternal primary hyperparathyroidism (MPH) is usually detected clinically in the first 2 weeks of life. Occasionally, diagnosis of primary hyperparathyroidism in a young asymptomatic mother is made when the infant presents with hypocalcemia. We present an infant with late onset hypocalcemia resulting from a combination of transient hypoparathyroidism due to ...
